This Mosquito Complaint Form is to assist the residents of communities served by the Franklin County Board of Health Mosquito Management Program (please see the listing above) to register a complaint or mosquito concern. If you are unsure of the city, village or township you live in please select “unknown” from the Community drop down menu. Please include as much detail as possible in the comment section. All complaints will be processed in the order received and attempts will be made to contact the submitter within one (1) business day.
