Form Instructions

All forms are in PDF format. You will need Adobe Acrobat Reader or the more recent Adobe Reader in order to view and download these forms. If you do not have Acrobat Reader or Adobe Reader installed on your computer, click the icon below to download a free copy of the latest Adobe Reader.

If you are having trouble displaying PDF files in your browser, please upgrade your Adobe Reader or follow the instructions below for saving the form to your computer.

When you click on a form name, a new window will open up; it may take a moment or two for the form to appear. Once the form appears, you have the option of printing it (by clicking the gray PRINT box on the top of the form), filling it out online and then printing it, or saving the form to your computer.

If you choose to fill it out online, click your cursor in the first line and simply type the information in. Then press the TAB button on your keyboard to move the cursor to the next line. In some cases, an option is required to be checked; press the lower case "x" key on that line to choose the appropriate option (for instance, if a question requires you to indicate ____yes or _____no).

After you have completed the form, print it out by clicking the PRINT box on the top of the form, or by clicking the printer icon at the top left of your screen. The gray box will not appear on the actual printed form.

To save a copy of a form to your computer, right-click on the name of the form (you do not need to open the form) and select "Save Target As..." if you are using Internet Explorer, or "Save Link As..." if you are using Firefox. Then choose where you want to save it on your hard drive - you may wish to save to your Desktop for easiest access. After the form is saved, you can fill it out and print it off as described in the previous paragraphs.
